This form helps us learn more about your organization, your client population, your curriculum needs, and the type of support you’re looking for.
After we receive your form, our team will review your information and email you a link to schedule your Provider Partnership Call.
For Counselors, Therapists & Treatment Professionals
If you are a counselor, therapist, or credentialed treatment professional interested in joining the Empowered Recovery Provider Network or learning how to use the Empowered Recovery framework in your professional role, please complete our Provider Network Inquiry Form.
This helps us understand your background, credentials, current work setting, and how you would like to integrate Empowered Recovery into your services.

For Coaches & Aspiring Recovery Coaches
